SMS Communications Policy

When you enroll in the Appa Health mentorship program by completing our intake form, you have the option to consent to SMS communication.

SMS communications are optional. During enrollment, you may choose YES or NO on the SMS opt-in question. Choosing NO does not affect your ability to participate in the Appa Health program - your care team will reach you by phone or email instead. If you choose YES, you may revoke consent at any time by replying STOP.

By selecting YES on the SMS opt-in question during enrollment, you agree to the following:

- You will receive text messages from Appa Health staff (mentors, navigators, and clinical team) about program coordination, scheduling, Google Meet session reminders, and check-ins related to your participation in our youth mentorship program.

- Message frequency varies, typically 1-10 messages per week per recipient depending on program phase.

- Message and data rates may apply.

- Reply STOP at any time to opt out.

- Reply HELP for assistance.

We do not share mobile information or opt-in data with third parties or affiliates for marketing or promotional purposes.
